In a metaphorical sense, it's used to mean something sweet, which is why Sanji continues to use it around beautiful women, most often around Nami and Robin. There's no specific reason why he uses this term, and it's likely a term of endearment, though more often than not it makes him look silly and makes us laugh.

Why does Sanji say "Mellorine"? What does it mean?
Sanji is known as one of the greatest Simp characters in anime; arguably, he is the greatest, and is certainly (alongside Pokémon's Brock and Demon Slayer's Zenitsu Agatsuma) the most famous Simp character in anime. Sanji has one major weakness: women. While his attitude may seem chivalrous when protecting pretty girls, the latter completely turns their heads and never fails to woo them, no matter the circumstances.
